SPECIFIC & GENERAL GIFTS (Identify in detail each specific item gifted, identify the person or organization to receive the gift, & explain any contingencies. (For ex.: I leave my vacation home to my mom, Dear Mom; I leave $500 dollars to Jane Jones). * This field is required.
RESIDUARY ESTATE (This is the remainder of your estate) This will be everything except for specific & general gifts. (Ex. I leave the remainder of my estate to my son. If my son is not living, I leave the remainder of my estate to my nephew, John. * This field is required.
BENEFICIARIES (List out your Beneficiaries).
FIDUCIARIES (Please name the person you desire to fulfill each of the following roles Ex. Personal Representative, Guardian, etc.) * This field is required.
Name your Personal Representative (PR). The PR is the person who administers your estate), List Address and Phone Number?
Name your Alternate #1 PR and List Address and Phone Number?
Name your Alternate #2 PR and List Address and Phone Number?
Name other Possible PR Alternatives (if you think this may be needed-list name, address and phone number)?
Name Guardian of minor children? (List address and phone #)?
Name Alternate Guardian of minor children? (List address and phone #)?
Have you discussed with each nominee (i.e. Personal Representative/ Guardian) of the potential appointments of each nominee?
Durable Power of Attorney (DPOA)---Do you want a Durable Power of Attorney? (A DPOA nominates a person to make all (financial/legal) decisions for you in the event you become incapacitated.)
Name your (DPOA) Agent and list Address and Phone # (A DPOA Agent is the person who makes decisions for you in the event you become incapacitated?)
Name your Successor Agent for your DPOA and list Address and Phone #
Do you want to limit or modify any powers of your (DPOA) Agent?
Do you want your Agent to have gifting powers? If so, do you want the gifting powers limited to certain persons or to a certain amount per year?
LIVING WILL ---Do you want a living will? ( A living will is a written legal document that spells out medical treatments you would not want to be used to keep you alive if you are unconscious or terminally ill.
DESIGNATION OF HEALTH CARE SURROGATE--- Do you want a designation of Health Care Surrogate? (A designation of Health Care Surrogate nominates a person to make your medical decisions in the event you become disable?)
Who do you nominate as your Surrogate Agent? List Address and Home Phone #, Work Phone # & Cell Phone #.
LIST ALL REAL ESTATE---List each piece of real estate owned, provide the address, legal description, folio, and how it is titled.
Do you have automobiles?
LIST OF ALL MONETARY ASSETS-- List all liquid, cash, investment, retirement, etc assets.
Do you own art?
Do you own Jewlery?
Do you have a safe deposit box(es)?
